This screening offers you an opportunity to know your health status, improve your lifestyle, and stay healthy.

Friday, January 5, 2024 5:00 a.m. — 8:00 a.m.

Tuesday, January 9, 2024 5:00 a.m. — 8:00 a.m.

Tuesday, January 16, 2024 5:00 a.m. — 8:00 a.m.

Friday, January 26, 2024 5:00 a.m. — 8:00 a.m.

· Blood Pressure

· Height and Weight

· BMI (Body Mass Index)

· Waist Circumference

· Tobacco Status

· Blood Glucose (blood draw)

· Blood Cholesterol (Total Cholesterol, HDL, LDL, Triglycerides, Cholesterol Ratio) (blood draw)

· Chemistry panel (blood draw)

· Colon Cancer detection take-home kits, optional for those over age 50

· PSA (blood draw), optional for men 50 and older or those with a risk for prostate cancer

COST:

Basic Screening: Free for all Employees; $45 for Contract Employees

PSA blood test: Free for all Employees; $22 for Contract Employees

Colon Cancer detection kits: Free for all Employees; $9 for Contract Employees

Twelve (12) Hour fast is required for blood work
