Friday, August 18, 2023 8:00-9:00 a.m. Princeton *ET (Eastern Time Zone)

Monday, August 21, 2023 6:30-7:30 a.m. 3rd Avenue

Tuesday, August 22, 2023 7:30-9:15 a.m. French Lick

Wednesday, August 23, 2023 6:30-9:30 a.m. Jasper

Thursday, August 24, 2023 8:00-9:00 a.m. Paoli

Friday, August 25, 2023 8:00-9:00 a.m. Washington

What screenings will be included?

· Blood Pressure

· Height and Weight

· BMI (Body Mass Index)

· Waist Circumference

· Tobacco Status

· Blood Glucose (blood draw)

· Blood Cholesterol (Total Cholesterol, HDL, LDL, Triglycerides, Cholesterol Ratio) (blood draw)

· Chemistry panel (blood draw)

· Colon Cancer detection take-home kits, optional for those over age 50

· PSA (blood draw), optional for men 50 and older or those with a risk for prostate cancer

· TSH (blood draw), optional

· CBC (blood draw), optional

· A1C (blood draw), optional

Who is it for? All Employees and Spouses

Do I need to fast?

Yes—12 hours. Take medication as prescribed. (We will schedule individuals with diabetes early if requested.) Drinking water is permitted and highly encouraged.

COST:

Basic Screening: Free for all employees and spouses

PSA blood test: Free for all employees and spouses (men)

Colon Cancer detection kits: $9 (optional)

TSH: $14 (optional)

CBC: $14 (optional)​​

A1C: $20 (optional)

¨ To keep you informed of your health status.

¨ To encourage you to practice healthy habits.

¨ To monitor improved health from year to year.

Appointments will be made every 15 minutes. Twelve (12) hour fast is needed for blood work.
