What screenings will be included?

Blood Pressure, Height & Weight, Body Mass Index (BMI), Waist Circumference, Cholesterol, HDL, LDL, Cholesterol Ratio, Triglycerides, Blood Glucose, Chemistry Panel lab test, and Tobacco status. Needle draw (venipuncture) is used to obtain blood for Lipid Profile and Chemistry tests. An optional PSA screening (blood test) will also be available to men 50 and older or those at a high risk for prostate cancer. An optional colon cancer take-home screening kit will also be available. Colon screening is recommended annually for adults 50 and older. Additional optional blood tests will include a Thyroid-Stimulating Hormone (TSH) test, Complete Blood Count (CBC) test, and Hemoglobin A1C test.

Do I need to fast?

Yes—12 hours. Take medication as prescribed. (We will schedule individuals with diabetes early if requested.) Drinking water is permitted and highly encouraged.

Why do screenings?

¨ To keep you informed of your health status.

¨ To encourage you to practice healthy habits.

¨ To monitor improved health from year to year.

How long does it take?

About 15 minutes. Appointments will be made every 10 minutes.

What does it cost?

The cost of the basic screening will be paid by the company for all insured and uninsured employees. Insured Employees: All additional tests are paid for provided that waiver to release information to insurance is signed at screening. Uninsured employees: Additional testing is self pay; Men that take the PSA blood test will need to pay an additional $22, and the colon cancer kits are an additional $9. The TSH and CBC blood tests are $14 each, and the A1C blood test is $20. The fees for these additional tests will be collected the day of the screening.

Are my screening results confidential?

Yes, only you and the screening staff at Memorial Hospital and Health Care Center will know the results. We encourage you to share the results with your health care provider.

When do I get my results?

Tuesday, April 16 07:00 a.m. – 12:00 p.m. (Corporate)

As requested, the above screening result session is reserved as a one-on-one. All screening participants are strongly encouraged to attend a Screening Result Session to pick up their results and have their results explained to them by a Health Educator. Screening packets that are not picked up after a Screening Result Session will be left in sealed, confidential envelopes with a company representative that you designate.

What if my results are outside the normal range?

Results outside the normal range present you with a Health Risk. The more risks, the greater the chance for cardiovascular disease and other illnesses. We recommend you consult your health care provider when this occurs. An extra copy of your results will be provided in your packet so that you may take a copy of your results with you to your next health care provider appointment. This screening does not take the place of routine medical exams.
