Some of my demographic/personal non-medical information is wrong. How do I correct this?
If you notice an error in your personal information, such as address or phone number, please send us a change request through the patient portal. The change request can be found by clicking Profile, and then Update Profile. Medical information cannot be updated through the patient portal. Medical information such as current medications and/or allergies, can be updated at your next visit when data is updated.

How do I communicate my medical concerns?
MyHealth is not used to communicate medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 911 OR YOUR PHYSICIAN'S OFFICE IMMEDIATELY.
How do I see my test results?
Your test results and reports are available by clicking on Health Record, located on the portal home page. Once you arrive on the Health Record page, look on the right side of the page, locate and click Results. You will then have access to your laboratory results. In addition your general reports can be located under Reports, Note: Laboratory results are available 36 hours after resulted. Reports are available after resulted and signed.
How do I get the complete set of my medical records?
You can obtain your medical records by contacting the Health Information Management department by calling 812-996-0537 (Monday-Friday, 7:00 a.m. – 3:30 p.m.).
I don't have an E-mail address, how do I get one?
Any E-mail service is compatible with MyHealth and there are many free E-mail services available to use; some of which are listed below:
How can I update my E-mail address in MyHealth?
To change the E-Mail address that MyHealth uses for sending activity, log into the portal and click on the Preferences tab at the bottom of the Homepage. Then click on the Update User Preferences Tab on the right hand side.

What information can I see in MyHealth?
You can see information from your medical record such as laboratory test results, medications, allergies, medical history, current health issues and pending appointments. You may also print a Health Summary for your records.
Is there a fee to use MyHealth?
No! MyHealth is a free service offered to Memorial Hospital and Health Care Center patients.
We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal ID’s and passwords. MyHealth also uses the latest encryption technology. All MyHealth communication is done while you are securely logged onto our website.
Can I also see my family members’ health records?
Yes, this is called Proxy Access. For a child under 18, this allows a parent or legal guardian to login to their personal MyHealth account, and then view information regarding their child. Per State and Federal guidelines, after the child reaches age 18, parents/guardians will need an additional authorization signed by the child to have access to the child’s medical record information.
It is also possible to request access to another adult’s health record if you help manage that adult’s medical care. For more information please contact the Health Information Management department at 812-996-5252.

What if I receive an E-mail with my one time user name and password and am unable to login to the portal?
The first thing you need to do is to make sure you enter the user name and the one time password exactly as it appears in the E-mail message you receive. The best way to assure that they are exact is to copy and paste from the E-mail message to the appropriate fields on the portal login screen. If that is not feasible and you must re-type the user name and password, know that the fields are case sensitive. Sometimes a lower case L (l) looks like a numeral 1 and an upper case O can look like the numeral 0. These simple issues can make it difficult to login the first time. Once you do login, you can change your password to something easier to remember and use. If you are sure you have entered all the characters exactly as they appear in the E-mail sent to you but are not able to login, contact the Health Information Management department at 812-996-5252 or send an email message to firstname.lastname@example.org.
